You defraud the company
You have not paid the premium
Your license is revoked or suspended
 
What to do if not pleased with insurer?
In case you are not pleased with the agent tell him or her about it. If still the complaint persists then just check if there is a department of consumer complaint and then file the complaint against the insurer.
If the issue is not solved, try to contact the state department of insurance, they are there for consumer protection and education and will offer you with resources and help in taking the complaint to the next level or provide you a new insurer.
